At InTouch Home Care, our nurses and therapists help you or your loved one live safely and more independently in the comfort of home after an illness, injury, or surgery. Our mission is to maximize your independence and full recovery through our supportive, safe, and compassionate care.

Our staff coordinates with your physician, family, caregivers, and community services in order to deliver the highest quality of care where you want to be cared for most – in the comfort of home.

Our approach includes a comprehensive assessment resulting in a unique treatment plan that you and your family participate in and is overseen by your physician. We understand that each individual is unique, and we are committed to providing a treatment plan that meets those individual needs.

The treatment plan will include:

  • A fall risk assessment and home safety evaluation
  • Instruction on all of your medications and potential side effects
  • Education on your medical condition and methods of self management
  • Communication with your physician on your progress toward your treatment goals
  • Skilled nursing and/or therapy treatment

 

Our focus is on patient education, primary care collaboration, medication reconciliation, assistance with equipment and home modification recommendations, and the provision of care as ordered by your physician. The benefits of patient centered care are that it greatly enhances clinical outcomes, reduces re-hospitalizations, increases patient satisfaction, and promotes greater communication with your physician.

Our nurses help you to take control of your treatment and make changes that may prevent future declines in your health. By working collaboratively with other members of the healthcare team they improve the safety in your home and prevent unplanned hospital visits through their care, which may include:

  • Wound care
  • Catheter care
  • Pain management
  • CHF & COPD management
  • Diabetes management
  • Medication education and administration

Assists you in learning how to restore function through:

  • Balance coordination activities
  • Therapeutic exercises
  • Home safety evaluations
  • Pacing & breathing activities for energy conservation
  • Fall prevention education
  • Pain management
  • Multi-level gait training
  • Bed mobility
  • Function transfers Instruction on safe & appropriate use of adaptive equipment

  • Helps you to regain the skills needed for the job of daily living. They train on the proper use of rehabilitative equipment & overall proven techniques to:

    • Improve ability to cook and eat
    • Improve ability to dress and groom yourself
    • Energy conservation
    • Improve basic motor functions and reasoning abilities
    • Compensate for permanent lack of function.

    Helps you cope with and recover from speech, language, and swallowing disorders through:

  • Treatments including oral exercises
  • Instructive or repetitive practice
  • Audio-visual aids
  • Dietary changes and gestural and reading/writing skills.

  • Assists you and your family with the emotional needs of your illness into an overall treatment plan through:

  • Community resource planning and coordination
  • Advance directives
  • Crisis intervention
  • Assistance with applications for financial aid, and teaching regarding coping with loss and change.

  • Are trained and certified to provide a wide range of personal care services such as:

  • Bathing
  • Grooming
  • Skin care
  • Personal hygiene,and activities of daily living.
  • (These services are available only in conjunction with nursing or rehabilitation services)
